For insurance purposes, each Member Club of the Folk Dance Federation of California, South, Inc. must keep an attendance log at each meeting or class. Everyone present must sign in. These logs must be retained for five years by each Member Club. The insurance company reserves the right the examine the logs. Clubs may use this form or any other convenient method of keeping an attendance log.                                         Form L - 12/03
